Care Communication Networks are networks of Home and Community Care, Health and Residential Care providers involved with the care of Clients, Patients, Residents and their Carers and Families.

Care Communication Networks recognise that Clients, Patients and Residents receive care and services across multi-care domains spanning Home and Community, Health and Residential Care. Improved communication and coordination and delivery of services will optimise the use of resources which is vital to the health and wellbeing of all people.

Each individual agency or healthcare provider maintains information relating to the person but all other agencies in the network are aware of where and what types of information are held on a particular person. The result is a coordinated view of the person and the services they have received/are receiving which supports an efficient and effective model of care and service delivery and case management.

Governance, privacy and consent protocols determine what specific information on person can be shared with others in the network.

Care Communication Networks are compliant with and enable programs such as SAFTE, Compaks, Community Options Projects, Single Point of Access, the Human Services Network Better Services Delivery Program, the Way Forward strategy as well as other strategies and programs to be implemented more easily and more effectively.
Care Communication Networks lead to more efficient use of human and financial resources; more effectively coordinated care and smoother transitions for Clients, Patients and Residents in and between different care settings such as Community Care, Hospitals, and Residential Care; as well as shorter length of stay in hospital. All delivering improved health outcomes and reduced cost of care.
