ICN Health has developed a unique, community driven care model and communication Platform, a Care Communication Network; that allows Community Care and Service providers to integrate the knowledge they have on their Clients with other Care and Service Providers including Area Health, Private Hospitals and GP’s etc in a Local area.

Through an easy to use client-centric viewer, agencies can work together optimally in full knowledge of who is servicing a client, what services are being provided, what data exists for that client, and any changes in the status of that client’s care and services.

ICN Health provides solutions for:

By bridging agreed data in agencies’ existing Client Management Systems into the Care Communication Network, a community knowledge net is created on the client and other service providers. There is no duplicate data entry and messaging other agencies to access more detailed information is simple and secure through the software interface.

A Care Communication Network is complementary with existing/planned systems and care models, such as NSW Health and DADHC , other electronic health and care records, Referral Information Centres and Single Points of Access etc. It does not replace them but integrates with and adds value to them by contributing to a continuous care record.

The benefits of being connected to a Care Communication Network include lower search costs, better collaboration between providers and less duplication in effort leading to improved health outcomes and reduced cost of care.
